Group Life Insurance Proposal Fact Sheet
 

Summary:

Halliday Life and Health recognizes the increasing need of Local 804 members requesting life insurance coverage during the COVID pandemic. This proposal will provide $25,000 of guaranteed life insurance for all Local 804 members working 10+ hours per week. The rate is $0.87 per week, regardless of age or health class, and paid by payroll deduction. The insurance is provided by UNUM, with Halliday Life and Health servicing the plan.  

At the July 26 General Membership meeting, there will be a voice vote by the members to accept the proposal. With an affirmative vote by the membership, the program will begin with an open enrollment period. During open enrollment, members can increase the guaranteed life insurance benefit up to $150,000 and add spouses with a guaranteed maximum benefit up to $50,000, at the same rate of $0.87 a week per $25,000 of benefit. 

If a member wishes to decline the coverage, they can do so any time during or after the open enrollment period. If a member opts out of the plan, and then wishes to be reinstated in the plan, a medical questionnaire will be needed and a new rate may be issued. The new rate may be equal or higher to the group rate.

For plan participants that wish to increase coverage, or for new union members that qualify for the benefit after the initial enrollment period, there will be an annual enrollment period to accommodate these members.

Estimated Schedule:

June – July 2020, Halliday Life and Health will provide marketing materials and educational resources to help members be aware of the benefits offered.

July 26, 2020: General Membership meeting to vote on the proposal.

August – September 2020: Open enrollment period

November 2020: Insurance becomes active
